Fairview Pharmacy Services: Fairview has provided comprehensive medication management (CMM) services (referred to as MTM) in their clinics since 1998. We currently have 80 MTM pharmacists.  Each year, our team has over 50,000 patient visits.

Other Fairview Pharmacy Services areas include: retail pharmacy, hospital pharmacy, long-term care pharmacy, compounding pharmacy, specialty pharmacy, home infusion pharmacy, clinical trials and Fairview’s own PBM, Clearscript.

Educational Experience

A typical work week includes 4 days in direct patient care in our primary care (family and internal medicine) clinics. The resident will work collaboratively with the clinic care team to address patient’s medication related concerns. Residents will conduct MTM visits in person or using telehealth technology (telephone and video). In addition to providing CMM services, the resident will be available to answer drug information questions (dosage recommendations, formulary medications, etc.) and research medication topics to assist clinic staff. Each resident will develop their own collaborative practice agreement with the providers at their clinic. Additionally, residents are given the opportunity to precept APPE students during their clinic time.

In addition to direct patient care, the resident will also complete a one-month management rotation that will extend longitudinally throughout the year (one day per week). The management rotation will expose the resident to system-level pharmacy projects, population health, contracting with health insurers, and the financial management of a large MTM program. Additionally, on this non-clinic day, the resident will also devote time to working on the residency project, management projects, creating a monthly pharmacy-related update to educate providers, and completing activities set forth by the University of Minnesota (Academic Day, Curbside Consult, etc). There is also the opportunity to help teach the Building an MTM Practice elective course at the University of Minnesota. Residents will also have a staffing requirement at the Riverside discharge pharmacy.

Allyson Schlichte, PharmD, MBA, BCACP is a manager of Ambulatory Pharmacy Services for Fairview Pharmacy Services. Allyson graduated from Drake University with a Pharm.D. and M.B.A. She has been providing care for patients since she started at Fairview as an Ambulatory Care resident in 2006. After residency, Allyson started a new MTM practice and clinical pharmacist position at the Fairview Maple Grove Medical Center and is currently providing MTM services at the Fairview Uptown Clinic. As a manager, Allyson works directly with Fairview IT to improve documentation for MTM as well as medication-related tools for the system. Allyson manages pharmacists working in population health to improve Medicare Star Measures and directly with insurance companies to help secure payment for MTM services. Allyson also serves as a preceptor for University of Minnesota APPE students and was named University of Minnesota College of Pharmacy Residency Preceptor of the Year for 2017-2018.

Amanda Brummel, PharmD, BCACP serves as the Vice President of Clinical Ambulatory Pharmacy Services.  Dr. Brummel has been employed by Fairview Pharmacy Services since 1999 when she graduated from the University of Minnesota.  Dr. Brummel has responsibility for the Medication Therapy Management (MTM) Program, the Anticoagulation Program, the Outcomes department, Compliance department and the clinical development and integration of ambulatory pharmacy services in the M Health Fairview Network. Dr. Brummel is also an Adjunct Professor at the University of Minnesota. She teaches the elective course, “How to Build an MTM Practice” and also co-developed an on-line CE course on building an MTM practice. She has published multiple articles on MTM and pharmacy’s role in the care team.  She has chaired and served on multiple committees and is a current member of the Minnesota Pharmacists Association, the American Society of Health-System Pharmacists and Pharmacy Quality Alliance (PQA).

Nicole Paterson, PharmD, BCPS is a Medication Therapy Management Practitioner at Fairview Pharmacy Services. Nicole graduated from The University of Nebraska Medical Center College of Pharmacy and completed an ASHP Accredited Pharmacy Practice Residency at Duke University Medical Center in North Carolina. Nicole has been a Medication Therapy Management Provider since 1998 and has started MTM at 5 different M Health Fairview Clinics, including her current location at M Health Fairview Clinics - Eagan. She continues to participate in the Diabetes and Cardiovascular Suite Care Map work. Nicole serves as a preceptor for University of Minnesota College of Pharmacy APPE students. Nicole was named University of Minnesota College of Pharmacy Residency Preceptor of the Year for 2011-2012.

Heidi Burres, PharmD, BCACP  is a Clinical Pharmacy Manager and
Medication Therapy Management (MTM) Pharmacist for Fairview Pharmacy Services. Heidi graduated with her PharmD degree in May 2010 from the University of Minnesota, Duluth campus. She started with the Fairview MTM program as an Ambulatory Care resident in 2010. Since completion of her
residency, Heidi has started MTM practices at several different clinics.  Heidi
currently provides MTM services at the Edina clinic and is a manager for the
MTM department. Heidi also serves as a preceptor for UMN APPE students
and a mentor for pharmacy students.

Michelle Wong, PharmD, BCACP has been a Medication Therapy Management Provider for M Health Fairview since 2018. After graduating from the University of Minnesota in 2018, she completed her ambulatory care residency with M Health Fairview and now practices at the M Health Fairview Apple Valley clinic. She was involved with Primary Care Transformation when first started at the Eagan and Apple Valley clinics and continue to be involved in these projects. She also enjoys precepting for University of Minnesota APPE students.

Danielle Bender, PharmD, BCACP has been a Medication Therapy Management (MTM) Pharmacist for Fairview Pharmacy Services since 2018. After graduating from North Dakota State University in 2017, she completed a one-year ambulatory care residency through the University of Minnesota at Bethesda Clinic and Walgreens. Currently, Dani practices at the M Health Fairview Roselawn Clinic. Dani also serves as a preceptor for University of Minnesota APPE students. 

 

Picture of Ann M. Philbrick, PharmD, FCCP, BCPS, BCACP

Ann M. Philbrick, PharmD, FCCP, BCACP is an Associate Professor at the University of Minnesota College of Pharmacy Department of Pharmaceutical Care and Health Systems and Adjunct Associate Professor at the Medical School, Department of Family Medicine and Community Health. She graduated in 2006 from the University of Nebraska Medical Center and completed a PGY1 and PGY2 residency in Ambulatory Care at the University of Iowa Hospitals and Clinics. Upon finishing her residency, she joined the faculty at the College of Pharmacy and Bethesda Clinic. Dr Philbrick has a passion for making professional connections with patients and loves to impart this passion onto pharmacy students and residents as well as medical students, physician residents, and faculty physicians. Her teaching, research, and patient care activities focus on the role of a pharmacist in an interprofessional setting.  She is an active member in the American College of Clinical Pharmacy, where she has served in several leadership positions.

Ila Harris, PharmD, FCCP, BCPS has been a Medication Therapy Management (MTM) provider at M Health Fairview Clinic - Bethesda Family Medicine since 1998. She is also Professor at the University of Minnesota Medical School in the Department of Family Medicine and Community Health and is on the faculty at Woodwinds Family Medicine Residency Program. She received her BS Pharmacy (1991) and PharmD (1993) from Philadelphia College of Pharmacy and Science and completed a specialty residency in Family Medicine at the Medical University of South Carolina. She was previously faculty at St. Louis College of Pharmacy. Ila has been a pharmacy residency preceptor for 13 years, for both the Bethesda Clinic/Walgreens and Pharmaceutical Care Leadership residency programs at the University of Minnesota Ambulatory Care/Postgraduate pharmacy residency program. She conducts practice-based research and has published numerous articles. She is active in the American College of Clinical Pharmacy (ACCP) and has served on the Board of Regents. Ila has received the U of M College of Pharmacy Outstanding Preceptor award, the Medical School Department of Family Medicine and Community Health Faculty Teacher of the Year award and the Teacher of the Year award from St. Joseph’s Family Medicine Residency Program.
